The ECI task force considers that much can be learnt from reviewing historical accidents and incidents – for many years examples have been passed between task force members in confidence and in good faith. A selection of these have been made available below.
Whilst attempts have been made to verify these accounts and avoid malicious reports, it should be noted that the ECI cannot accept responsibility for their content or their accuracy.
How to search
The search uses keywords in the document title, and you can enter more than one keyword to search for by separating them with a comma.
You can also specify a category to search within, and which type of files (images of pdf documents) you want to search for by checking the appropriate boxes/buttons.
