Learning from Accidents

The ECI task force considers that much can be learnt from reviewing historical accidents and incidents – for many years examples have been passed between task force members in confidence and in good faith. A selection of these have been made available below.

Whilst attempts have been made to verify these accounts and avoid malicious reports, it should be noted that the ECI cannot accept responsibility for their content or their accuracy.
